TERM AND CONDITIONS

The enrolled Program Participant receives a 30% rebate off a single submitted dealer invoice up to $30,000 for HuFriedyGroup branded products only. Rebate does not apply to shipping and taxes.

Offer expires five (5) years after the date of graduation. Proof of graduation is required at time of order. Minimum order of $5,000 retail value (before rebate, tax, and shipping fees) required. The certificate is only redeemable by the named dentist. The certificate is not transferable and cannot be combined with any other offer. Special and Made-to-Order items are not eligible for a rebate. Offer valid in U.S. only.

The enrolled Program Participant must be starting a new practice or joining a multi-doctor practice. Corporate DSO and group practice orders subject to authorization based on evaluation of existing agreements.

* To redeem the certificate, contact your HuFriedyGroup Representative or call 1-800-HuFriedy. Orders must be placed with an authorized HuFriedyGroup dealer and involve a HuFriedyGroup Regional Account Manager. At the time of redemption, the resident will need to submit the rebate request form and provide a copy of the dealer invoice and a copy of the issued HuFriedyGroup certificate.

You qualify if you are a 1st year practicing dentist, and graduated from a specialty program. The following specialty programs qualify:

• Endodontist - Periodontist - Oral Surgeon - Prosthodontist - Pedodontist - Orthodontist - AEGD Qualifications
• Redeem within 5 years of completion of specialty program.
• Proof of specialty diploma
• Completed online resident rebate form
